NowPow’s Chicago Network Continues Rapid Growth With Addition of Catholic Charities
CHICAGO, October 28, 2019 – NowPow, a personalized community referral solution, today announced Catholic Charities of the Archdiocese of Chicago, one of the largest private, nonprofit social service agencies in the Midwest, will become the latest to join its Chicago Network.
The Chicago NowPow Network, which is connecting health and community care across Chicago, includes more than a dozen cornerstone health systems, community health providers and payers, as well as an array of community-based organizations (CBOs). The Network has more than 1 million patients in its database and is on pace to become one of the nation’s largest multidirectional community resource referral networks.
Catholic Charities will initially participate in the Chicago NowPow ecosystem as a referral partner, receiving NowPow’s Personalized Community Referrals for patients who have health-related social needs such as food insecurity or housing issues. Referred clients will benefit from Catholic Charities’ extensive network of services and a holistic approach to care.

NowPow’s referrals are highly personalized to reduce access barriers. The technology uses multivariable matching that incorporates demographics, condition codes, screening results, risk factors and more. It also allows organizations to filter referrals at the individual service level by location, eligibility requirements, language(s) spoken, age, gender, service restrictions, and other critical access factors.
Health systems who refer patients to Catholic Charities will be able to coordinate and track referrals as appropriate among those at high risk. That means they can readily monitor activity and outcomes at the individual, organization and network-level to optimize care. Both the referral sender and receiver will also be able to nudge patients to access these community services and ultimately close the loop on care.
